OCD Treatment ERP, Medication Management, and TMS

TMS therapy session at Turnwell Mental Health

OCD is often misunderstood, and that misunderstanding leads to treatment that makes it worse rather than better. Reassurance and avoidance feel like relief in the moment, but they reinforce the cycle. The approaches that actually work, ERP therapy and appropriately dosed SSRIs, require a provider who understands the disorder well. Turnwell offers both, with TMS therapy available as a next option for OCD that has not responded to first-line care.

Understanding OCD

OCD Is Not Just About Cleanliness or Order

Obsessive-compulsive disorder is characterized by obsessions (intrusive, unwanted thoughts that cause distress) and compulsions (repetitive behaviors performed to temporarily reduce that distress). OCD presentations vary widely and include contamination fears, harm obsessions, symmetry and order, religious or moral scrupulosity, and sexual or aggressive obsessions, among others.

ERP (Exposure and Response Prevention) is the gold-standard psychotherapy for OCD. It involves systematic, clinician-guided exposure to feared situations without engaging in the compulsive behavior, building tolerance and reducing the anxiety response over time. High-dose SSRIs are the standard medication approach, often at doses higher than those used for depression.

For patients who do not respond adequately to ERP and medication, TMS therapy is FDA-cleared using a deep TMS protocol specifically indicated for OCD. Turnwell offers TMS for OCD at select locations. A clinical evaluation determines which approach is most appropriate for your presentation.

Frequently Asked Questions

Your Questions Answered

What is OCD?
Obsessive-compulsive disorder is a psychiatric condition characterized by intrusive unwanted obsessions and repetitive compulsions performed to reduce distress. OCD presentations vary widely and are often misunderstood. A psychiatric evaluation establishes the diagnosis and guides treatment.
What is the most effective treatment for OCD?
The gold-standard treatments for OCD are ERP psychotherapy and high-dose SSRIs, typically used in combination. For OCD that does not respond to these, TMS therapy using a deep TMS protocol is FDA-cleared as a next treatment option.
Is TMS therapy covered by insurance for OCD?
TMS for OCD is covered by many major insurance plans when medical necessity criteria are met, typically requiring documentation that ERP and medication have not produced adequate improvement. Turnwell verifies your OCD TMS benefits and handles prior authorization.
How is OCD different from generalized anxiety?
OCD is distinct from generalized anxiety in its structure: obsessions are ego-dystonic (experienced as intrusive and unwanted) and compulsions are performed specifically to neutralize obsessions. The response to treatment also differs. An accurate diagnosis guides appropriate treatment selection.
